Certificate for the transportation of human remains to BiH

For repatriation of human remains from the Republic of Poland to Bosnia and Herzegovina, it is necessary to submit the following documents:

1. If the deceased was a holder of a valid BiH passport, did not die of any contagious disease, and had not been buried in the Republic of Poland, the Embassy may issue the certificate for the repatriation of the deceased’s human remains without a consent from the competent bodies of BiH upon submitting of the following documents:

  • Filled out application form requesting the certificate for the transfer of human remains;
  • Copy of the deceased’s valid BiH passport;
  • Medical Certificate of Death with the notarized translation in one of the official languages of Bosnia and Herzegovina (either Serbian, Croatian, or Bosnian);
  • Death Certificate (international form);
  • Certification from the Funeral Home that the human remains had not been buried and that the casket, with the remains, has been prepared for shipment according to the international standards;
  • Proof of a reserved burial place;
  • Information about the Funeral Home’s vehicle, the driver and the place of entrance into Bosnia and Herzegovina;
  • Consular fee payment receipt.

2. If the deceased was not a holder of a valid BiH passport or the cause of death was a contagious disease or the deceased had been buried or cremated in the Republic of Poland and the remains are being transported after exhumation, the following documents should be submitted:

  • Filled out application form requesting the certificate for the transfer of human remains;
  • Filled out request for the transportation of the remains from abroad to BiH, depending on the place of interment (Federation of BiH, Republika Srpska, Brčko District BiH);
  • Medical Certificate of Death with the notarized translation in one of the official languages of Bosnia and Herzegovina (either Serbian, Croatian, or Bosnian);
  • Death Certificate (international form);
  • Certification from the Funeral Home that the casket, with the remains, has been prepared for shipment according to the international standards;
  • Proof of identity – it can be any personal document with a photo.
  • Proof of a secured burial place.
  • Information about Funeral Home’s vehicle, the driver and the place of entrance into Bosnia and Herzegovina
  • Consular fee payment receipt

In such case that the repatriation of human remains is organized after exhumation, the certificate is issued only after receiving an approval from the competent bodies of BiH.
